To be effective, your written notice must include all of the following:
On receipt of a valid notice we will act expeditiously to remove or disable access to the identified material, and we will make reasonable efforts to notify the affected user. Where the material is supplied to us by an upstream provider, we forward the notice to that provider and require them to act.
Incomplete notices will be acknowledged with an explanation of what is missing. We cannot act on vague reports that do not identify specific material.

We may restore the material after 10 to 14 business days unless we are notified that court proceedings have been filed.
We maintain a policy of terminating, in appropriate circumstances, the accounts of users who are determined to be repeat infringers. Accounts terminated on this basis are not eligible for a refund.
Knowingly making a material misrepresentation in a notice or counter-notice may expose you to liability for damages, including costs and legal fees, under section 512(f) of the DMCA. Please be certain before you submit.
